I acknowledge that Muay Thai and martial arts training involve physical contact, inherent risks of injury, and strenuous physical activity. I voluntarily assume all risks associated with participation.

I agree to release, waive, and hold harmless Tier One Muay Thai, its coaches, staff, and affiliates from any and all claims, damages, or injuries arising from my participation.

I confirm that I am in adequate physical health to participate and will inform my coach of any medical conditions or injuries prior to training.

I agree to follow all gym rules, coach instructions, and training protocols at all times.
